You will also need to furnish the following items to register a child for school.

1. Certified birth certificate from the State (copies from the hospital or County Health Departments are not valid.)
2. Social Security Card (original, no copies)
3. Blue Immunization Form
4. Two verification of residence documents (power bill, or other utility bill showing current address and guardian name.)

Guardianship papers if there is joint custody or primary custody of student. (copies will be made and kept in student's permanent record file for future reference to visitation rights at school or permission to check out the student.)
